Preferred Provider Organization (PPO)
By paying a monthly premium with PPO, you are covered for a percentage of dental costs. Members pay a deductible. You can obtain savings of up to 35% and you will not be charged more than the contracted rate (even if the dentist’s normal rate is higher) by an in-network dentist. Guardian Dental’s PPO has 363,000 locations with 124,000 dentists.
In-network dentists send claim forms on your behalf. You also have the option to select an out-of-network dentist, but you may be charged regular rates and Guardian will only subsidize the standard amount (according to them), so you may end up being responsible for the difference.
There is no deductible for preventative care when using an in-network dentist and $50 deductible for all other dental care. There is a maximum dollar pay-out annually which rises annually. Waiting periods vary according to the service provided.
